CHAMPS at Ashley County Medical Center

The following documents will be needed to complete the application. You will be able to upload the documents as word or pdf documents within the application. Forms that require signatures must be signed and not typed. Students applying for CHAMPS should have a minimum 2.5 GPA.

• Transcript (can be obtained from school counselor)
• Counselor Recommendation Form (1 required)
• Teacher Recommendation Form (1 required)
• Photo Release
• Parent/Guardian Consent Form
• Confidentiality and Hold Harmless Agreement
