Getting Started

If you are an active patient, you can create a Patient Portal account. You will need a Personal Identification Number (PIN) to create your on-line account.

Requesting Your PIN

During your office visit, you can request Patient Portal Personal Identification Number (PIN) from the front desk or your nurse. You will be required to provide a valid photo ID. You will be asked to read and sign an “Access Request Form.” A Secure PIN Letter will be given to you during your visit or you may request on be mailed to your registered address.

Secure PIN Letter

This letter will include your unique and secure Personal Identification Number (PIN) and will guide you through your new account registration process. Your PIN will only be used one time during the Identification Verification process of creating your new account. When creating your new Portal account, please make note of your Username and Password. You will need these logon credentials to access our Patient Portal services after registration.

E-mail Requirements

Each patient is required to have a unique email address to use the Patient Portal and Secure Messaging. If you do not have a personal email address already, you can obtain a free email address from many services such as: Gmail:, Hotmail:, Yahoo:
Please note, a spouse, family member or legal guardian may not share one email address. Each patient must create a separate account.

Secure Messaging

You will receive a notification e-mail in your e-mail Inbox. This notification e-mail will include a link to your Patient Portal Secure Message In-box. Follow the link, log on to the Patient Portal account and you will receive your confidential (private) message.
